Returns one row per clinician per city/ZIP with their practice addresses there (phone, group practice and PAC ID, assignment, telehealth), plus the exact total (up to 20,000). Needs last_name, state or zip_code. Specialty matches the PRIMARY specialty only (list values with clinician_count_by_specialty). For a group practice by name use clinician_search_groups."
